Request an Appointment

If you aren’t sure if your diagnosis is eligible for treatment with medical cannabis, you can fill out the form with the cannabis clinician we have affiliated with and they will get in touch with you to determine eligibility. Or if you know you are definitely eligible you can schedule an appointment with one of their providers. Their knowledgeable staff will help you get the support you need to know whether or not you qualify.

 

No prior diagnosis, nor medical records, for undiagnosed PTSD are necessary. Texas laws allow the treatment of the an array of symptoms that come with PTSD. Common symptoms such as insomnia, aggression, intrusive thoughts, and anxiety have been shown to be improved with cannabinoids. Our cannabis clinician affiliate can administer a PTSD assessment during your appointment to determine if you qualify. Anyone can do a similar assessment on themselves using the MyHealthEVet web assessment tool.In Texas, the following conditions qualify a patient to be in the TCUP program:

 

  • All epilepsy and seizure disorders
  • Multiple sclerosis or spasticity
  • Incurable neurological disorders (Alzheimer’s, Parkinson’s, etc.)
  • Autism
  • Amyotrophic lateral sclerosis (ALS)
  • All Cancer patients qualify, not just those will terminal cancer
  • Post traumatic stress disorder

 

Medical records for all other qualifying conditions are required to get a prescription. You will be asked to submit your medical records prior to your appointment with medical providers. See below about how to obtain records.

Step 2:

Complete Virtual Visit With Certified Cannabis Clinician

In this consultation, one of the clinical providers will meet with you virtually to discuss why you are wanting medical cannabis and your chief complaint/symptoms. If you qualify, you will be given a prescription based on the best product for your needs. With our partnered affiliate clinician, your prescription will be good for 6 months.

Know Before You Go

 

 

  • Make sure to upload proof of your diagnosis to your account prior to your appointment (not applicable to undiagnosed PTSD).

  • You can request your past medical records by calling or emailing the doctor’s office that made the diagnosis. If you are a veteran, one of the quickest and easiest ways to get ahold of your records from the VA is by having access to My Healthevet, which is the VA’s medical site for veterans. Veterans can access records from the HEALTH RECORDS section, and even message their providers requesting that if a record they have submitted is not clear on a diagnosis, to enter a clarification into the record.

Step 3:

Complete Product Purchase Direct With Dispensary

Once your consultation is over, you will be entered into the CURT registry and contacted by a dispensary within one business day following your appointment. The dispensary will help you find the best product options for you based on your current prescription(s). The dispensary will give you three options for receiving your product:

 

 

  1. Pickup at the dispensary in Austin, TX.

  2. Pay a delivery fee to have the paid prescription product delivered to your house using a courier service.

  3. Pickup from one of the dispensaries’ drop-off locations. Our affiliate clinician has a store and is a pick-up spot for the greater Fort Worth area. There are other drop off spots provided by the dispensaries on a routine basis. See the list of spots for one near you.

Step 4:

Schedule Follow Up Visits Typically Every 6 Months To Renew Prescription

Our affiliated clinician usually writes prescriptions for 6 months at a time, you will need to have recurring follow-up appointments every 6 months at minimum to keep your prescription active. This is a great time to assess how you are doing and if there needs to be a change in your prescription. You are free though to schedule an appointment earlier if you feel the need to discuss a change to your prescription.
